How to Order Your PER4MAX Chair

Select the order form for your chosen model below. Forms are available as fillable PDFs — complete them digitally, or print and fill by hand.

Take Your Measurements
Accurate measurements are critical to building a chair that fits perfectly. Use our Measurement Videos for step-by-step guidance. The order form will show you exactly what is needed. If you need assistance, contact us to schedule an appointment.

Fill Out Your Form
Fill out as much as you can — measurements, colors, configuration options, accessories, contact information, and any special requests. We'll ask you to provide at least 3 photos of you in your current or borrowed wheelchair.

Submit For Quote And Follow-Up
Fill out our 'Get a Quote' form, submit your photos and order form (optional). Our team will review your details and follow up quickly with next steps, or to schedule a free consultation.

Yes. All PER4MAX order forms are fillable PDFs — you can complete them on any computer, tablet, or phone. Save your completed form and email it directly to our team at sales@PER4MAX.com. Please send over at least 3 photos of you in your current or borrowed wheelchair as references for our team. You can also use our custom chair configurator to build and design your chair, just make sure to save and submit your design to our team via email or our Get a Quote form.

Each order form includes a measurements section with clear labels for every dimension we need. For guidance on how to take each measurement accurately, watch our Measurement Videos — they walk you through every step and help ensure your chair fits exactly right.

Our team reviews your form and follows up within 1–2 business days to clarify any questions, or schedule a virtual/in-person consultation, before finalizing your quote. This is to ensure we design your chair to your unique needs and specifications. Once the order is confirmed and a deposit is received, production begins. Lead times vary by model and configuration — your sales contact will provide an estimated timeline.

Contact us as soon as possible if you need to make changes. We generally schedule an initial virtual/in-person consultation to dial in your specifications prior to processing your order. Modifications can typically be accommodated before production begins. Once a chair is in production, significant changes may affect your lead time or quote.

Yes. PER4MAX works with VA hospitals as well as CAF and other grant and funding sources. Contact our sales team to discuss your funding situation before placing your order — we'll help you navigate the process.

Lead times vary depending on the model and your configuration. Each wheelchair is custom designed, built by hand, and tested prior to delivery. Your sales contact will provide a production estimate once your order is confirmed. We'll keep you updated throughout the process.
